1966 Annual - William Hartnell
Doctor Who Annual 1966, was the first Doctor Who Annual to be released. Published by World Distributions it was released in September 1965.
The front cover and spine simply say 'The Dr. Who Annual' with no mention of the year.
The Annual features the First Doctor (William Hartnell) and has none of the companions and has appearances by the Zarbi, enoptera, Sensorites and Voord.
An electronic (PDF) version of this annual and an audio reading of the story, The Lair of Zarbi Supreme by William Russell, were both included as extra's on The Web Planet DVD.
Note, that in this Annual The doctor is referred to as 'Dr. Who' throughout.

Fifteen Page prose story featuring The Doctor and Gordon Hamilton. The doctor finds himself back on Vortis, after receiving a radio signal about earth and must rescue groups of Humans and Menoptera from an intelligent Zarbi power.
The Sons of the Crab

Second story featuring the Doctor, Formal (The Chief Yend), Mernogil (Yend Biologist), Valkro (Yend Biologist). The doctor arrives on the planet Wengrol, in the Crab Nebula, and is taken as a specimen by humanoid scientist who believe that the Doctor is a shapeshifter. In this story The doctor mentions that this is the furthest that the Tardis has travelled.
The Lost Ones

The third story sees The Doctor arrives on a mysteries planet and decides to investigate his surroundings. He is taken prisoner by the Menoptera who think he is one of the enemy that have been attacking them.
The Monsters From Earth

The forth story was a thirteen page prose and featured the Sensorites and the Zilgan as the enemy. Amy Baker and her brother Tony, accompanied by their dog Butch, play hide and seek and use the Tardis as a place to hide. While exploring the Tardis they accidently become stowaways.
Peril in Mechanistria

The fifth story sees the Tardis Space-Time Locator damaged following an escape from the Daleks on Scaro. The Doctor cannot accurately pilot The Tardis and is happy when it finally lands on a mysterious metal planet.
The Fishmen of Kandalinga

The sixth and final short story sees The Tardis materialise at the bottom of a large body of water on a mysterious planet. This story occurs, from The doctor and The Voord's perspective, after the events of The Keys Of Marinus
